  • Duration:  Part Time Fixed term until 30 June 2025
  • Base Salary:  $113,390- $123,993 per annum (RN 4.1- 4.3 or Admin 7.1- 7.4)

WHO ARE WE?

Established in 1991, Danila Dilba Health Services (DDHS) is a not-for-profit, Aboriginal community-controlled organisation that is committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) region in the Norther Territory.

We are focused on acute care, immunisation, chronic disease management, women’s health, men’s health, child and maternal health and sexual health.

We also provide a range of community programs in areas including health promotion, mental health, social and emotional wellbeing, and alcohol, tobacco, and other drugs.

ABOUT THE ROLE

This position is responsible for clinical leadership and advice of quality, culturally sensitive Care Coordination for Complex Chronic Disease clients at Danila Dilba Health Service. The Care Coordination team leader will support the streamlining and coordination of integrating Care Coordinators as one team and ensure policies, procedures and work instructions are maintained and match service delivery. The team leader will support Care Coordinators with complex clients and support DDHS services to improve Care Coordination systems and processes. The team leader will have a clinical caseload and contribute to Care Coordination service delivery.

WHAT YOU WILL DO

  • Provide supervision and operational support for care coordinators across Danila Dilba Health Service
  • Monitor service delivery workflows and management of caseloads within Care coordination.
  • Complete reporting for DDHS and funding bodies, taking accountability to ensure the content is true and completed by the required deadlines.
  • Provision of Care Coordination to clients with Complex chronic conditions as identified within the clinics.
  • Coordinate and develop DDHS processes, policies, and procedures to ensure Care Coordination is equitable across DDHS.
  • Contribute to effective clinical governance arrangements that ensure high levels of quality and safety and evidence-based practice.
  • Maintain quality client medical records.
  • Contribute to strategies that strengthen chronic disease care and ensure it is responsive to local needs.

SELECTION CRITERIA

Essential

  • Qualification as Registered Nurse or Aboriginal Health Practitioner and current Registration with AHPRA.
  • Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions and relevant management strategies.
  • Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and wellbeing of Aboriginal and Torres Strait Islander people.
  • Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.
  • Demonstrated experience in project management and Continuous Quality Improvement in a healthcare environment.
  • Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
  • High level of personal resilience and flexibility with the ability to deal with challenging situations.
  • Demonstrated computer skills especially in the use of client information systems or similar.

Desirable

  • Experience providing supervision and leadership to a multidisciplinary team.
  • Experience working in the Aboriginal Community Controlled Health sector.
  • Experience working in a clinical practice or community health education program.
  • Minimum 2 years’ experience as a Care Coordinator.
