WHO ARE WE?

Established in 1991, Danila Dilba Health Services (DDHS) is a not-for-profit, Aboriginal community-controlled organisation that is committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) region in the Northern Territory.

We are focused on acute care, immunisation, chronic disease management, women’s health, men’s health, child and maternal health and sexual health.

We also provide a range of community programs in areas including health promotion, mental health, social and emotional wellbeing, and alcohol, tobacco, and other drugs.

WHAT WILL YOU FOCUS ON?

Culture Care Connect (CCC) is a proactive Aftercare service that is designed and delivered to support Aboriginal and Torres Strait Islander People following a suicide attempt or suicidal crisis.  Culture Care Connect provides a safe environment, culturally appropriate care co-ordination, support and linkages with different aspects of treatment or response.

RESPONSIBILITIES

  • Provide high quality counselling input to Aboriginal and Torres Strait Islander individuals who have been referred following a suicide attempt or suicidal crisis.
  • Provide high quality clinical leadership and oversight for Healing and Recovery Service including intake triaging, risk assessments for all referred clients
  • Perform clinical risk assessments case by case
  • Lead and provide co-ordination of care into tertiary mental health services.
  • Provide expert clinical and other expertise relevant to Aboriginal health issues to the delivery, evaluation and improvements of the service
  • Lead case reviews with Healing and Recovery team and external providers as required.
  • Maintain strong referral partnerships, including but not limited to the following services: acute care services, primary care services, schools, police, Alcohol and Other Drugs Services mainstream and Aboriginal and Torres Strait Islander-led mental health services.
  • Provide clinical oversight and input into the development of group activities, procedures and work instructions for the program delivery.
  • Undertake reporting on aftercare activities in line with Service Agreement requirements including ensuring appropriate data collection.
  • Maintain client confidentiality.

QUALIFICATIONS

Essential

  • Qualifications in Social Work/ Mental Health Nursing/ Counselling or Psychology/ Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care along with current registration with AHPRA or relevant association.
  • Membership of Australian Association of Social Workers/ Australian Counselling Association.
  • Demonstrated awareness of and sensitivity to Aboriginal culture and history; and an understanding of social, health and wellbeing issues affecting Aboriginal and Torres Strait Islander families.
  • Excellent written and oral communication skills.
  • Demonstrated understanding of relevant legislation and policy frameworks.
  • Experience in the delivery of culturally safe care
  • Client focused with an ability to advocate on client’s behalf.
  • Ability to multi-task, prioritise workloads and meet deadlines.
  • Proficiency in computer software including electronic medical software and Microsoft Office Suite.
  • Ability to work independently and in a team environment.

Desirable

  • Aboriginal and Torres Strait Islander descent.
  • Experience working in suicide prevention.
  • Experience working in aboriginal community-controlled sector.
