Australian Family Partnership Program (AFPP)

Supporting first-time mums throughout the first two years of parenthood.

The Danila Dilba Australian Family Partnership Program (AFPP) is a home visiting program that supports Aboriginal and/or Torres Strait Islander first-time mums throughout their pregnancy (from 16 weeks) and into the first two years of their parenting journey. The program offers continuity of care and social support, with our clients confident that the team are only ever a phone call away.

The AFPP team includes:

  • Four nurse home visitors
  • Four family partnership workers
  • A nurse supervisor

The team regularly visit women in their homes, or a venue of their choice, and provide a wide range of social supports. This includes accompanying clients to antenatal and obstetric appointments, social support, assistance navigating external services like Services Australia and housing issues, and assisting with the purchase of baby items.

Those enrolled in the AFPP program are invited to participate in a group catch-up every two months. This allows them to meet other families and children and start building a strong support network.

“I never felt forced or pressured to come, but I knew the ladies were there if I needed them,” past client, Melissa said. “I don’t know what I would’ve done without their support.”
