Position: Transient and Outreach Aboriginal Health Practitioner (AHP)

Location: Darwin Clinic         

Duration: Full time, fixed term until June 2025

Classification: $87,809 to 96,681 per annum (AHP 3.1 to 3.4)

This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT).

Conducting thorough health assessments and addressing immediate needs, the AHP collaborates closely with a multidisciplinary team to ensure a comprehensive approach to health and social support.

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We work to address the social determinants of health including the broader issues that affect our people’s health, through evidence-based research and public advocacy. We also work to empower individuals, families, and communities to optimise their health.

WHAT WILL YOU FOCUS ON?

In the Transient and Outreach Service aimed at vulnerable homeless and transient individuals in the greater Darwin region, the Aboriginal Health Practitioner (AHP) assumes a multifaceted role focused on holistic care provision and support. Tasked with conducting thorough health assessments and addressing immediate needs, the AHP collaborates closely with a multidisciplinary team to ensure a comprehensive approach to health and social support. Emphasising continuity of care within the Darwin region, the AHP facilitates smooth transitions for individuals departing the area, or accessing other services, advocating for their health and social welfare.

RESPONSIBILITIES

  • Conduct thorough health assessments to identify immediate and ongoing health needs.
  • Stabilise urgent health conditions and provide necessary medical interventions and treatments.
  • Collaborate closely with a multidisciplinary team to address both health and social support requirements.
  • Develop and implement individualised care plans tailored to each patient's unique needs and circumstances.
  • Facilitate access to essential healthcare services, including primary care, mental health support, and substance abuse treatment.
  • Advocate for patients' rights and access to quality healthcare within the healthcare system and community.
  • Provide culturally sensitive care that respects the diverse backgrounds and experiences of patients.
  • Ensure continuity of care for individuals within the Darwin region, coordinating with relevant healthcare and social service providers.
  • Support individuals transitioning out of the region by connecting them with appropriate resources and services in their destination areas.
  • Participate in ongoing program evaluation and quality improvement efforts to enhance service delivery and outcomes for vulnerable populations.

QUALIFICATIONS

Essential

  • Qualification as a health professional (e.g., Aboriginal and Torres Strait Islander Health Practitioner) and current registration with AHPRA.
  • Minimum 3 years’ experience working in a Primary Health Care setting.
  • Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and wellbeing of Aboriginal and Torres Strait Islander people.
  • Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and / or cardiac disease) and relevant management strategies.
  • High level of personal resilience and flexibility with the ability to deal with challenging situations.
  • Demonstrated competence working within multi-disciplinary teams or independently, with strong organisational skills and the ability to plan, manage and prioritise tasks.

  • Current Working with Children Clearance (Ochre Card) or the ability and willingness to obtain one
  • Current CPR certificate or the preparedness to gain one within 6 months of commencement. 
  • Full vaccination against Covid-19 or evidence of exemption as listed on the Australian Immunisation Register (AIR)
  • Current drivers’ licence
  • Work right documents (Australian passport/birth certificate OR international passport with visa).
