About the Role
Danila Dilba is seeking an experienced and motivated Transient & Outreach Program Coordinator to lead the Transient & Homeless Team and Mobile Outreach Team. This leadership role is responsible for coordinating culturally safe, high-quality primary health care and social support services for people experiencing homelessness, unstable housing, or transience in the Darwin region. The successful candidate will strengthen integration between outreach services and primary health care clinics, lead a multidisciplinary team, coordinate care across internal and external services, and drive improved access, continuity of care, and health outcomes for clients with complex health and psychosocial needs.
What You Need to Succeed
- 3-5 years of experience in similar field
- Leadership and/or management experience
- Experience in housing and/or community services
- Well-developed knowledge of, and contemporary experience in, working with clients who experience transience or homelessness
- Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and well-being of Aboriginal and Torres Strait Islander people.
- Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.
- Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
- High level of personal resilience and flexibility with the ability to deal with challenging situations.
- Demonstrated computer skills, especially in the use of client information systems or similar.
