Chronic Disease Services
Danila Dilba offers a range of services designed to support clients living with a chronic condition or disease. Our Chronic Disease team consists of:
- Allied Health
- Care Coordination
- Rheumatic Heart Disease (RHD)
- Sexually Transmitted Infections (STI) / Blood Borne Viruses (BBV)
- Aboriginal Disability Liaison Officer (ADLO) program
- Tackling Indigenous Smoking (TIS) program

Allied Health
The Danila Dilba Allied Health team work with other clinical staff to provide targeted services to help our clients to live full, happy and healthy lives.

Allied Health services include:

Physiotherapy
Our physiotherapists are experts in how the body moves. They work with clients of all ages and treat injuries linked to sport, diabetes, weight issues, osteoarthritis and stroke.
Our physiotherapists also run a weekly hydrotherapy group and a regular DDHS Gym Group at Charles Darwin University (CDU) gym.

Dietetics
Our dietitian can help pregnant mums, babies, children, adults and older people to eat well and meet your individual nutritional needs i.e. iron deficiency.
Our dietitian also works with adults and children living with diabetes, heart disease or who have weight concerns to support you to make health choice to improve your overall quality of life.

Podiatry
Our Podiatrist is an expert in foot care and can show clients how to care for their feet and prevent disease.
Clients with diabetes should see a podiatrist every year – and more often if they are living with foot damage caused by diabetes, falls, arthritis, or have muscular or circulatory diseases.

Oral Health Therapist and Dental Services
Danila Dilba offers a suite of dental services to eligible clients through a partnership with NT Health. The clinic operates out of our Palmerston Clinic two days per week. To receive these services, clients need to be referred by their clinic.

- Preventative services
- Fillings
- Baby teeth extraction
Our oral health therapist is an expert in dental health and supports clients to prevent tooth decay and manage oral health concerns.

Care Coordination
Many of Danila Dilba’s clients live with one or more chronic disease which require ongoing support and regular allied health support services. Our Integrated Team Care (ITC) team is made up of qualified health professionals (called care coordinators) who provide one-on-one care coordination to support you if are living with a chronic disease if you have received a GP Management Plan from your doctor.
Your care coordinator also works closely with your GP and your allied health team.
How can a Care Coordinator support me?
Care Coordination includes (but is not limited to):
- Helping you get to appointments
- Involving your family and carers in your care when you need us to
- Supporting you to learn more about and manage your condition
- Clinical monitoring, recalls and follow up
- Assisting you to:
- Stick to your care plan
- Learn the best ways to manage your chronic condition
- Connect you with other services who can also support you
Am I eligible?
You need to be a DDHS client and referred by your doctor to be eligible to receive ITC services.

Aboriginal and Disability Liaison Officer (ADLO) program
The Danila Dilba Aboriginal Disability Liaison Officer (ADLO) program funded by the National Disability Insurance Agency (NDIA) to assist Aboriginal and Torres Strait Islander people living with disabilities to become active participants of the National Disability Insurance Scheme (NDIS).
Our Danila Dilba ADLO staff support clients to access the NDIS via:
- Identification of a potential NDIS client
- Assistance to obtain assessments to determine NDIS eligibility
- Communication with the NDIS to submit application
- Advocacy and support to clients to ensure their NDIS plans suit their individual needs
- Advocacy for clients already accessing the NDIS to ensure they receive appropriate supports and care to support them to successfully live in the community.

Next time you visit Danila Dilba, tell our clinic staff that would like some ADLO support. Our team can then refer you to the ADLO team.
Our ADLO will then contact you to arrange a time to come and meet and yarn about your situation and the supports you need.

Tackling Indigenous Smoking (TIS)
Danila Dilba’s TIS program helps Aboriginal and Torres Strait Islander people develop skills to quit smoking and vaping through tailored and culturally safe services, including one-on-one consultations, school and community visits and health promotion activities in the community.
The TIS team deliver smoking education sessions at middle schools in the Darwin and Palmerston regions. They team also run smoking cessation sessions for adult groups, including at residential rehabilitation centres and for pregnant clients who attend Danila Dilba clinics.
The team also run education sessions to promote smoke-free environments and conduct health promotion stalls at community and sporting events and in shopping centres.

Vaping and the Environment
Like smoking, vaping has negative health effects caused by a range of toxic particles. Vapes are also harmful to the environment. The short-term, disposal devices can cause fires and leach metals, plastics and chemicals into our land and waterways.
Vaping is a Fool’s Game – Rap Music Video
Danila Dilba partnered with Clontarf Academy and Stars Foundation in 2024 to create a rap music video to warn young mob of the dangers of vaping.
